Instructions:
There are several ways to complete this Information Technologies (IT) Survey. The first and preferred way is to go to the Orient’s webpage and complete the form. It can be found as a google form. The second option is to download a version of the IT Survey form [MS WORD], fill it out, and then email it back to it-survey@scottish-rite-mn.org. The third option is to print out this survey [PDF], fill it out by hand, and mail it to the Minneapolis Valley at the following address.

Whichever method you pick, complete as much information as you know and send it to us. Audit Timeline:
10/1/2023 – Send out IT Audit form to Valleys.
10/30/2023 – Deadline to return the forms
11/15/2023 – Complete the direct contact and review with each Valley
11/30/2023 – Provide a summary and suggestions to the SGIG.
